Create your first form
Build a form in the visual builder, then publish it.
You can describe a form to the agent, or build it yourself in the builder. This page covers the builder.
Make a form
- Click New form in the sidebar. The builder opens on an empty form.
- Name it at the top left, or click its title on the page and type. People see the name at the top of the form.
- Click Add question in the dock at the bottom (or press
/) and pick a type. Hover the space between two questions and click + to add one right there. See Question types. - Type straight into a question's label and help text on the page. Click the question for the rest of its settings: whether it's required, its options and its validation.
- Open Form settings in the dock to pick how it flows: Classic, One question at a time, Multi-step or App. See Display modes.
- Open Design in the dock to choose its look; the page restyles as you go. See Design and themes.
- Click Preview at the top to try it as a respondent would.
Everything saves as you go. The status at the top shows Saving… and Saved, and you can undo and redo any change.
Publish it
Click Publish at the top right and confirm. The form is now live at its link. See Publish and share.
Good to know
- A form can be published once every choice question has options. The builder tells you what's missing if it isn't ready.
- Editing a live form doesn't change it for respondents until you click Publish changes. See Versions.
- Short on time? Start from a template on Home, or let the agent build a first draft.
